RHHS Athlete Registration Form

This registration information must be completed by the student's parent/guardian annually.  If it is determined that someone other than the parent/guardian completed the registration, the student will not be allowed to participate in Bryan County athletics.

Students must have the following in order to participate at RHHS.
1. Physical form completed by a doctor.
2. Electronic Heat and Concussion form submitted by student.

3. Sudden Cardiac Arrest form submitted by student/parent.

 

Physicals:

• It is a single PDF document

• It has Parental, Physician & Athlete signatures.

• It has 4 complete pages.

• Use an app like "Genius Scan" or "Notes" to scan a physical into a single PDF document. 

• No physical copies will be taken.
